Question 1 of 3

One question at a time. A focused sequence, with room for the instructions that go with each question.

Illustrated examples with fictional answers. Switch layouts to compare how a form is presented.

Shape the form around the person filling it out.

  1. Choose a display format

Use One question at a time for a focused flow, Single page for an at-a-glance form, or Multiple pages to group related questions and uploads.

  2. Choose how people answer

Go beyond text boxes. Use choices, scales, tables, repeating groups, drawings, and signing fields. Add instructions beside the questions they explain.

  3. Try it before you send it

Preview the selected layout with practice answers. Check the flow, instructions, and signing experience, then save the form for future requests.

Keep instructions close to the question
Informational passages accompany response controls without adding an extra conversational step. Longer forms can use named page groups to make related questions easier to follow.
Show the questions that apply
Use conditional visibility to show relevant questions or document requests based on earlier answers. Required fields and validation help recipients find what still needs attention.
Make room for review
Recipients can review their active answers and return to make corrections before submission. Packets retain each source form’s layout, so a consent and a questionnaire can use different formats.

A few practical answers.

Which form display formats are supported?

PHI Uploader supports One question at a time, Single page, and Multiple pages. Choose a format for each saved form. Multiple pages groups questions and uploads by named section, while packets preserve each source form’s selected layout.

What question types can I put in a form?

The editor offers 27 field and content types: short text, paragraph, number, email, phone, website, address, date, time, date and time, duration, checkbox, yes/no, dropdown, single choice, multiple choice, rating, ranking, matrix, table, repeating group, attestation, signature, initials, drawing, diagram markings, and instructions/content. Document upload requests are available separately.

Do I need AI import access to use the expanded fields?

No. The expanded controls are available in the manual form editor without AI import access. AI-assisted document import is a separate feature that requires account enablement.

Can I preview the recipient experience?

Yes. The form preview uses the selected layout and supports practice answers and signatures. Preview uploads are disabled, and finishing a preview does not submit a recipient response.
