From a blank document to a form you can send.

  1. Bring your existing form

    Upload one blank PDF or a set of JPEG or PNG images. Check the pages, put them in order, and rotate them before generating a draft.

  2. Review the draft

    AI identifies questions, instructions, consent passages, and document requests. Compare the draft with each source page, correct wording, and resolve flagged items.

  3. Make it your own

    Choose the question types and layout, preview the recipient experience, and save a reusable form. Add it to a packet or use it in a future request.

Review against the original
Source pages stay alongside the draft during review. Check every page and consent passage, and decide how to handle items that need correction or exclusion.

Review and refine choices, tables, rating scales, initials, signatures, and diagram markings. The expanded question types are also available when you build a form manually.
Organize before you send
Give questions meaningful page groups, select a display format, and try the preview. Saving the imported form adds it to your library; sending or publishing it is a separate step.

Which documents can I import?

Import one blank PDF or ordered JPEG/PNG images of an English-language form, up to 20 pages and 50 MiB per import. Use blank forms without patient information. Word documents are not supported directly; export them to PDF first.

Will the online form look exactly like my paper form?

The import creates editable form fields and content rather than an exact reproduction of the paper layout. Review the wording, choices, diagrams, and consent passages, then choose One question at a time, Single page, or Multiple pages for recipients.

Does AI review replace staff review?

No. AI produces a draft that can contain omissions or mistakes. Your team reviews every source page, corrects the fields, resolves flagged items, and confirms the consent wording before saving.

Is document import available on my account?

AI-assisted import currently requires account enablement in addition to the required workspace setup. Contact PHI Uploader to discuss access. You can use the expanded question types in the manual form editor without AI import access.
