One link for your whole intake packet.
Bring your saved forms together in a reusable document packet. Families follow one protected link through the questions, uploads, and signatures your team needs, then review and submit everything together.
A warmer welcome.
Less loose paperwork.
- 01Welcome & consentSingle page · signature included
- 02Intake questionnaireOne question at a time
- 03Documents & historyMultiple pages · upload requests
Build the packet once. Use it for the next family, too.
Choose your forms
Combine saved forms for intake, consent, records, or insurance updates. Give the packet a name and arrange its forms in the order recipients should complete them.
Send one request
Choose the recipient and Drive destination, then share the protected request link. Send the access code separately. Recipients do not need a Google or portal account.
Follow it through
Recipients complete each form and review the packet before submitting. Your team follows the request status through scanning and delivery of the PDF summary and clean uploads.
Keep the forms that work. Bring them together.
- Each form keeps its own format
- A short consent can stay on a single page while a longer history uses one question at a time. Packets preserve each source form’s chosen layout and signing controls.
- Answers, uploads, and signatures together
- Include required questions, file requests, initials, acknowledgments, and signatures. The recipient sees progress through the forms and a combined review before submission.
- Reuse without changing requests already sent
- Future assignments use your current source forms. Requests already issued keep the version they were created with, so an edit to your library does not change a family’s existing packet.
A few practical answers.
What is a document packet in PHI Uploader?
A document packet is an ordered collection of saved forms delivered as one request. It can include intake questions, instructions, document uploads, consent, initials, and signatures. Recipients complete the forms through one protected link and submit one combined packet.
How many forms can a packet contain?
A packet can contain up to 20 saved forms. Choose the forms and arrange their order in the packet composer. A form can be reused in different packets.
Can different forms use different display layouts?
Yes. Each form keeps its selected layout: One question at a time, Single page, or Multiple pages. A single-page form keeps its signature on that form’s page, and the packet ends with a combined review.
What happens after the packet is submitted?
Uploaded files are malware-scanned before Google Drive delivery. The generated PDF summary and clean uploads go to the configured Drive destination. The recipient receives a submission reference, and staff can follow processing and delivery status.
Give your next intake a clear beginning and end.
Start with the forms your staff sends together most often. Complete workspace setup, then bring them into one reusable packet.
